This blog will explain what bunions and bone spurs are, how they differ, the general causes, the effects on the foot and person, and how to deal with them for comfort. What is a bunion? To understand bunions, it's important to describe the bony anatomy of the foot.   There are five long bones just before the toes, called metatarsals. The first metatarsal bone connects the midfoot to the big toe. It is the strongest and thickest of the five metatarsals, as it bears more weight and force during gait. It is at this juncture that bunions are born.  A bunion is a bony bump on the inside (medial side) of the big toe joint - the connection between the first metatarsal and the big toe. The medical term is hallux abducto valgus, which means the big toe is bent toward the second toe and twisted, with the associated bump on the side of the big toe joint.  The bump is actually not an extra bone, but in fact the head of the first metatarsal sticking out. With a bunion, or hallux abducto valgus deformity, what occurs over time is that the first metatarsal slides out away from the second metatarsal. This protrusion from the foot gives the appearance of a bump of new or enlarged bone. To add to the bump, there may also be a bursal (fluid filled sac) over the bump, accompanied by soft tissue swelling and inflammation. For those dealing with this condition, finding the right footwear is crucial. Arthritis can result in osteophytes or spurs wherever there are joints. In the foot, a common area affected is the top of the midfoot, where patients often present with painful bumps.  Comparative analysis of bunions vs. bone spurs Bunions can develop due to a myriad of reasons, including biomechanical abnormalities in the foot that result in abnormal motion and increased pronation. This causes increased forces on the first metatarsal, leading to a gradual shift in its position over time. Other contributing factors include loose ligaments, genetics, and wearing ill-fitting shoes that squeeze the forefoot or elevate the heel. Particularly during pregnancy, choosing the right shoes is crucial. Shoes for pregnant women should provide ample support and room to accommodate changes in foot size and shape, reducing the risk of bunions and other foot-related issues. Bone spurs can also occur for many reasons. Spurs on the back of the heel often arise due to arthritis and increased pronation. Overpronation causes the calf muscles, which attach to the Achilles tendon, to overwork in an attempt to compensate. This constant extreme pull can stimulate the growth of extra bone.  Plantar heel spurs can also result from overpronation, as the plantar fascia, a ligament that stabilizes the foot, is stretched when the foot elongates during pronation. This stretching exerts tension on the attachment at the heel, potentially leading to spur formation. Generally, any stress on the heel, such as being overweight or arthritis, can contribute to the development of a heel spur. Understanding the differences between overpronation and underpronation is crucial, as each condition stresses the foot differently and requires specific approaches to management. Higher arches are particularly prone to these spurs because ground force can push back against the forefoot, causing jamming in the midfoot joints. This stress on the joints can lead to the formation of spurs. Again, arthritis can also contribute to their development.  How do I know if I have a bone spur or a bunion? Bunions can lead to issues with shoe fitting as the, making it increasingly difficult to find comfortable shoes. Prevention and care  When it comes to both bunion prevention and treatment options, wider and deeper shoes are a good starting point. In addition, the toe box should be round and not pointed. Shoes that are only wider shoes with a non-round toe box may not be enough to accommodate the bump. Shoes should also have a low or flat heel, as elevating the heel places stress on the big toe joint, which can make the bunion worse.  For heel spurs, every part of the shoes should be stable to control motion and pronation. This blog will explain what Baxter's nerve is, what an entrapment is, what causes it and how to treat it.  What is Baxter's nerve entrapment?  Baxter's nerve is a branch of a larger nerve known as the Posterior Tibial Nerve. It is situated in the tarsal tunnel, located just below the medial or inner ankle bone. The formal name is the Inferior Calcaneal Nerve, as it travels under the foot to reach the bottom of the heel or calcaneus bone. It is both a sensory and motor nerve. In other words it feels the sensations on the heel and along its course, and moves small muscles to the little toe and the outside of the foot.    Baxter's nerve entrapment causes Due to the way the nerve travels to the heel, it has to pass under a muscle (abductor hallucis)  and turn to run down to the heel. If the muscle is too large from overuse, swelling, or abnormal shape, the nerve can be compressed, squeezed, or entrapped.  Entrapment means too much pressure on the nerve, causing it to malfunction. If you ever had your arm fall asleep from lying on it too long, you know what an entrapment is and how it can feel. Symptoms of Baxter's nerve entrapment can include sharp burning pain on the inside of the foot just below the ankle, pins and needles on the side of the foot in the same area, numbness on the inside of the foot under the ankle, and pain on the bottom of the heel. The pain is usually worse after being on it for a while or walking or running, and gets worse the more time is spent on the foot. Tapping on the inside of the foot, below the ankle, will often cause pins and needles. How to tell the difference between plantar fasciitis and nerve damage? Baxter's nerve entrapment can be difficult to differentiate from plantar fasciitis, which also causes pain on the bottom of the heel in the same area. One difference between the two is that plantar fasciitis often hurts worse when first getting up from sitting or lying down and can improve after a while on it, while Baxter's is worse later after being on it.  Diagnosing Baxter's entrapment requires a good exam and sometimes an ultrasound or an MRI. It can fool even an experienced practitioner at times. In some cases, plantar fasciitis can exist with Baxter's nerve entrapment.  Baxter's nerve entrapment treatment Treatment can and should start with proper shoe wear and possibly inserts or orthotics. A common cause of Baxter's nerve entrapment is overpronation of the foot. This is where the foot rolls in and the arch usually lowers significantly.  Overpronation can enlarge or swell the abductor muscle which entraps the nerve. Also, it can compress the heel where the nerve ends up squeezing it. Reducing pronation is critical to helping treat this problem and the subsequent pain.  The role of footwear in managing Baxter's nerve entrapment Baxter's nerve entrapment shoes should have a solid heel counter and extra depth to handle inserts or orthotics, and reduce abnormal motion, and a low heel, wide roomy toe box and a stable midsole for motion control. This blog will deal with a very common foot deformity: Hallux abductus, the medical term for when the big toe is curved inward. It will cover the causes of this deformity and discuss what can be done to address it, including the importance of proper footwear/shoes.  Hallux abducto valgus is the term that describes bunion deformity. In effect, there is a bump on the side of the big toe joint while the big toe bends or curves toward the second toe. Hallux means big toe, abducto valgus means big toe curves toward the second toe, while it also twists or rotates (valgus). This is a common deformity, especially in women.  Less common is a simple curvature of the big toe toward the second toe without the bump or bunion on the big toe joint. When this occurs, the big toe often rides under the second toe, elevating or lifting the second toe and causing further issues as it rubs in the shoe.  What causes a big toe to be curved inward? Causes of this deformity are usually some type of biomechanical abnormality in the foot, causing the big toe to be forced into the floor while walking. The ground pushes back against the big toe, which then pushes it toward the second toe. Over time, this permanently deforms the toe into the curved position.  The underlying biomechanical issues are often genetically influenced. Other causes that can contribute to this condition are hypermobile or loose joints in the feet, pregnancy, which also loosens foot joints, or just abnormality in the way the foot moves.  In addition, the wrong footwear that compresses the big toe inward can gradually push it towards the second toe. Shoes that are too tight or narrow can cause this issue. How do I stop my big toe from bending inwards?  The most important way to slow down or reduce the chances of this deformity comes down to proper shoes, and sometimes orthotics or well-made inserts in the shoes.  While, in some cases, the deformity will occur despite all interventions, more often than not it can be slowed down or prevented from worsening. The only way to undo the deformity once it has occurred, is surgery. One key to minimizing the likelihood of developing or worsening this deformity is to manage abnormal forces in the foot and address the increased motion issues. In addition, as orthotics are often required in severe cases, the shoes should be extra deep and work well with orthotics.  The other important factor is proper shoe FIT. Untreated, it will progress to the breakdown of the joints and dislocation and permanent deformity of the foot.  Initial treatment is complete immobilization of the foot, ankle or lower leg in a cast or solid boot. In addition, the patient should be non-weight bearing until the foot cools down and stabilizes. This can take months in severe cases,” he says. What type of footwear is best for Charcot foot? “After the cast or boot is removed, the person can return to shoes, sometimes with a brace if deformity has damaged the foot structure. Shoes should be appropriate to the level of damage (if any) that Charcot has caused,” clarifies Dr. Geshman.  He goes on to specify that “If the foot is structurally sound and intact, then a well made supportive shoe is worn. A scooter that allows the affected leg to completely rest can be helpful.” He goes on to explain, “After the initial period, shoes and inserts that accommodate any deformity that has occurred are critical. Avoiding ulcers is the goal as once a wound or ulcer opens up, it can take months to heal and sometimes leads to amputation.” Does Charcot foot get worse? According to Dr. Gershman, it can. “If proper initial treatment is not followed and permanent deformity occurs, the foot is at high risk of ulcers. Ulcers are very difficult to heal in these cases as the underlying deformity, such as protruding bone on the bottom of the foot, is a constant factor. Also, diabetics don't heal well due to the disease itself. Infection is a constant risk as diabetics are very prone to infection,” he adds. “Charcot foot is a very serious condition, but can be managed if treated early and aggressively. The patient has to stick with the often difficult treatment plan. However, the end result of proper treatment is a fully functioning foot. If not, the condition will worsen and can lead to amputation,” summarizes Dr. Gershman.

The shoes need to be extra deep for the orthotics.” According to Dr. Gershman, many CMT patients require a brace or what is called an AFO (ankle-foot orthotic) that goes under the foot and up the lower leg. “Shoes need to accommodate this brace with extra depth and a roomy toe box,” he says. “Many shoes will work with an AFO. They also make a stretchable toe box shoe which works well with an AFO and is especially helpful for hammertoes,” advises Dr. Gershman. Over time, the pressure from the squeezing causes the nerve coating to thicken with scar tissue and causes pain and abnormal sensations, such as numbness or pins and needles feeling on the side of the foot or the bottom of the heel. It is often misdiagnosed as plantar fasciitis. What types of shoes are recommended for someone with a toe amputation? It depends on which and how many toes have been amputated and somewhat on the reason for the amputation.  Common causes for amputated toes include diabetes with either poor circulation or lack of feeling/sensation, or a combination of both. Most commonly these issues lead to an infection that can destroy the soft tissue on the toe leading to gangrene or lead to a bone infection called osteomyelitis which requires removal of the bone or entire toe.  Other causes of amputations can include trauma which causes major damage to the toe making it non-salvageable, smoking which reduces circulation to the toes, and clots in the blood vessels to the toes, again, blocking blood flow to the toes.  The most problematic toe to lose through amputation is the big toe called the hallux. When you walk as you lift the heel and push off in gait the big toe is where the final push off occurs. It also acts as a stabilizer in gait and when standing as it can grip the ground to keep balance.   When you lose the big toe, gait changes occur to compensate and the lesser toes can be stressed leading to deformities, pain and balance problems. Shoes become critical in compensating for loss of the big toe.  The shoe must be stable with non slip outsole, firm midsole to control gait, deep to allow an insert or orthotic to make up for the loss of the toe, have a wide toe box to reduce the chances of rubbing on the lesser toes causing further deformity and further amputations. The leather should be soft with no seams in the inside of the shoe and a fairly flat heel so as not to put additional stress on the remaining toes. If the amputation is in the middle toes and it’s only one toe, usually there are no immediate issues as the remaining toes compensate and shift slightly. As long as the shoe has a wide deep toe box to avoid pressure on the toes, there can be normal gait and reasonable results. "Some foot surgery necessitates avoiding all weight-bearing for a month or two; others are quite minor and may not require changing shoe wear at all." Dr. White explains that when there are stitches, a post-op shoe is usually worn for a couple of weeks until the stitches are removed. A post-op shoe is a specialized shoe designed to provide support and protection during the initial healing phase after surgery. “There is often associated swelling such that more casual shoes are best first worn,” adds Dr. White. “Orthofeet shoes, in particular, offer the benefits of having additional depth and also come with thin fitting spacers. What should I avoid after foot surgery? If your bones need to heal, it’s important to follow your doctors’ instructions regarding weight bearing. The actual duration depends on the location of the surgical site and can range from being able to walk right away (toe surgery) to no weight-bearing for eight weeks (rearfoot surgery).   In addition, if there are stitches, you have to prevent the foot from getting wet.  There is typically swelling following any foot surgery, so it’s necessary to wear roomier, stretchable shoes and avoid shoes that are too narrow or otherwise put pressure on the foot. What happens if you walk too soon after foot surgery? “Walking too soon after foot surgery may interfere with healing, whether it’s preventing a fracture site from maintaining proper alignment or a skin wound from closing up,” says Dr. White. “Walking too soon can also lead to prolonged swelling. Swelling can sometimes be controlled using compressive dressings, which are specialized bandages designed to apply pressure to the affected area.
